Extended Access Service:

SDHC successfully delivered 1,294 appointments to patients, ensuring timely access to care:

  • 375 face-to-face consultations
  • 919 telephone consultations

Without this service, patients indicated they would have sought alternative forms of care:

  • 48% would not have sought any medical assistance
  • 32% would have contacted NHS 111
  • 20% would have visited an Urgent Treatment Centre or A&E
 

Care Home Out-of-Hours (OOH) Service

This is an entirely remote service which operates every weekend. Each home has a 'check in' consisting of proactive calls to see if there are any problems where a GP can be telephoned for advice.

The Care Home OOH service provided 75 consultations for care home residents, resulting in significant system-wide benefits:

  • 74 calls prevented to NHS 111
  • 4 calls prevented to 999
  • 4 hospital admissions prevented

Vaccination Campaigns: COVID-19

Our vaccination Campaign efforts made a significant impact on community health:

  • Total: 711
  • Outreach off-site clinics: 200
  • Care Home visits: 124
  • Housebound visits: 387
 

Urgent Primary Care

SDHC has been actively supporting ESHT’s UTC with GP capacity, contributing to alleviating the pressures faced by both the A&E department and the onsite UTC.

Total Consultations Delivered: 429

If access to this service was not possible:

  • 24% would have contacted NHS 111
  • 29% would have waited to see their GP
  • 47% would have visited A&E
