Delivering Quality Care and Expanding Access
As a social enterprise, we are committed to reinvesting the income generated from our private GP services into enhancing the quality and accessibility of NHS Services within our community.
By choosing our private services, you help support a model that drives improvements in care delivery, expands access to vital services, and ensures we can continue offering the highest possible standard of care.

Extended Access Service
SDHC successfully delivered 1,134 appointments to patients, ensuring timely access to care:
- 361 face-to-face appointments
- 773 telephone consultations
Without this service, patients indicated they would have sought alternative forms of care:
- 42% would have called 111
- 9% would have attended Urgent Treatment Centres or A&E
- 46% would not have sought medical assistance
- 3% would have contacted 999
Care Home Out-of-Hours (OOH) Service
The Care Home OOH service provided 259 consultations for care home residents, resulting in significant system-wide benefits:
- 256 calls to 111 avoided
- 36 calls to 999 avoided
- 28 potential hospital admissions prevented
Top reasons for consultations:
- Complex frailty care (37.97%)
- Medication management (18.63%)
- Respiratory issues (11.08%)
- Urology concerns (7.08%)
- Dermatology needs (6.84%)


Vaccination Campaigns: COVID-19 and Flu
Our vaccination Campaign efforts made a significant impact on community health:
COVID-19 Vaccinations
- 3,731 total vaccinations delivered
- 893 care home patients vaccinated
- 514 housebound patients vaccinated
- 1,127 vaccinations administered through outreach events in underserved areas
- 1,197 vaccinations were administered to all other eligible cohorts.
Flu Vaccination Rollout
- 1,225 flu vaccinations delivered (partial rollout)
