SDHC Service Highlights - March 2025
Delivering Quality Care and Expanding Access

Extended Access Service
SDHC successfully delivered 1,446 appointments to patients, ensuring timely access to care:
- 454 face-to-face appointments
- 856 telephone consultations
Without this service, patients indicated they would have sought alternative forms of care:
- 51% would not have sought medical assistance
- 36% would have called 111
- 12% would have attended Urgent Treatment Centres or A&E
- 1% would have contacted 999
Care Home Out-of-Hours (OOH) Service
The Care Home OOH service provided 168 consultations for care home residents, resulting in significant system-wide benefits:
- 166 calls to 111 avoided
- 16 calls to 999 avoided
- 13 potential hospital admissions prevented
Top reasons for consultations:
- Complex frailty care (33%)
- Medication management (12%)
- Dermatology (6%)
- Urology concerns (11%)
- Palliative Care (6%)

Urgent Treatment Centre
SDHC has been actively supporting ESHT’s Urgent Treatment Centre (UTC) with GP capacity, contributing to alleviating the pressures faced by both the A&E department and the onsite UTC. For the month of March, SDHC provided 25 appointments per day 5 days a week, with a total of 134 appointments being utilised throughout the month.
We intend to continue offering this vital service, further streamlining operations, and supporting the local Emergency Department in managing demand.
