Delivering Quality Care and Expanding Access

As a social enterprise, we are committed to reinvesting the income generated from our private GP services into enhancing the quality and accessibility of NHS Services within our community.

Extended Access Service:

SDHC successfully delivered 1,346 appointments to patients, ensuring timely access to care:

  • 298 face-to-face consultations
  • 1048 telephone consultations

Without this service, patients indicated they would have sought alternative forms of care:

  • 50% would not have sought any medical assistance
  • 25% would have contacted NHS 111
  • 25% would have visited an Urgent Treatment Centre or A&E
 

Care Home Out-of-Hours (OOH) Service

This is an entirely remote service which operates every weekend. Each home has a 'check in' consisting of proactive calls to see if there are any problems where a GP can be telephoned for advice.

The Care Home OOH service provided 198 consultations for care home residents, resulting in significant system-wide benefits:

  • 192 calls prevented to NHS 111
  • 23 calls prevented to 999
  • 15 hospital admissions prevented

Vaccination Campaigns: COVID-19

Our vaccination Campaign efforts made a significant impact on community health: Total in May– 2435

  • Outreach off-site clinics: 1463
  • Care Home visits: 778
  • Housebound visits: 188
 

Urgent Primary Care

SDHC has been actively supporting ESHT’s UTC with GP capacity, contributing to alleviating the pressures faced by both the A&E department and the onsite UTC.
