SDHC Service Highlights - February 2025
Delivering Quality Care and Expanding Access

Extended Access Service
SDHC successfully delivered 1,182 appointments to patients, ensuring timely access to care:
- 481 face-to-face appointments
- 839 telephone consultations
Without this service, patients indicated they would have sought alternative forms of care:
- 51% would not have sought medical assistance
- 36% would have called 111
- 12% would have attended Urgent Treatment Centres or A&E
- 1% would have contacted 999
Care Home Out-of-Hours (OOH) Service
The Care Home OOH service provided 111 consultations for care home residents, resulting in significant system-wide benefits:
- 108 calls to 111 avoided
- 12 calls to 999 avoided
- 12 potential hospital admissions prevented
Top reasons for consultations:
- Complex frailty care (48%)
- Medication management (22%)
- Respiratory issues (13%)
- Urology concerns (10%)
- Palliative Care (10%)


Virtual Ward Service
SDHC is working in partnership with Brighton & Hove GP Federation (B&HF), and Alliance for Better Care GP Federation (ABC), on behalf of Sussex Community NHS Foundation Trust (SCFT), to deliver the Virtual Ward Service.
SDHC will be providing GP Clinical Decision Makers for the service in the High Weald, Lewes, and Havens (HWLH) area of East Sussex. The GPs work alongside the Urgent Community Response (UCR) team to provide seamless, high-quality care which enables patients to be treated in their normal place of residence, rather than being admitted to hospital.
